Payment is expected when services are rendered. As a courtesy we will file your primary dental insurance carrier for estimated payment, providing all necessary insurance information is given prior to your first dental visit. Estimated co-payments and any deductibles are due when services are rendered. If your insurance has not paid within 60 days you are responsible for the balance in full.
Secondary insurance can be filed for you. However, it is not accepted as a form of payment. Advance payment for treatment is non-refundable.
Late Charges
If the balance goes unpaid a late charge will be applied of 1.5% each billing month. Failure to keep the account current may result in termination from our practice. In the case of default on an account past 120 days your account could be subject to an outside collect agency in which you would incur an additional 40% fee that would be added to your account.
Office Courtesy
As a courtesy to other patients and staff we ask that all cell phones and pagers are turned off before entering treatment rooms

To our patients with Dental Insurance
Our office files your dental insurance for you and accepts assignment of your benefits as a courtesy to you.
We strive to make sure every effort is made to make you aware and help you understand your maximums, deductibles, and co-payments. While we try to be as accurate as possible these are only estimations of your benefits. Please keep in mind that your policy is a contract between you and your insurance company. If you do not follow your insurance plan terms they may not pay for part or all of your dental care. Our office works with hundreds of insurances on a daily basis and it is impossible to be 100% accurate. While we are happy to file these claims for you, knowing your benefits, keeping up with maximums, and knowing deductibles is ultimately your responsibility. If your insurance request any additional information from you please respond as quickly as possible so that their payment is not delayed. If you have secondary insurance, we will file this claim for you, but it will not be accepted as form of payment. Please be aware if you do have secondary coverage there is always a non-coordination of benefits clause.
